Apply for a new PAN card

Just fill up this form & follow the instructions below:

Instructions:

1. Take a print out of the form. The form cannot be saved, so you should take a print out once you have filled up the form. If you are having troubles filling the form online, just take a print of the blank form and fill up the form manually. You should use BLACK ink only and fill up using BLOCK letters.
2. Sign the form.
   • Individuals
     • The signature should be in BLACK ink only
     • The signature should be within the specified box (should not cross the box)
     • Sign within the box just below the Photograph Area (top right corner of Page1)
     • Sign within the box at the end of the form (bottom right corner of Page2)
   • Others (e.g. Companies, Firms, Trusts, HUFs etc.)
     • Requisite Rubber Stamp should be used (e.g. Director, Partner, Karta, Authorized Signatory etc. rubber stamp)
     • Signature should come within the rubber stamp and should also be within the box (top right corner of Page1 and bottom right corner of Page2)
3. Paste one Passport size photograph (applicable for Individuals only) in the Photograph Area (top right corner of Page1).
4. Attach Self attested copy of the ID proof (in case of companies, firms etc. the attestation should carry the required Rubber Stamp). Click here for valid list of ID proofs.
   • ID proof should clear depict 
     • Your name in Full (abbreviated names are not allowed e.g. If your name is Raj Abhay Singh then the ID proof should clearly mention the same and Raj A Singh or R A Singh is not allowed)
     • Your fathers name in Full
     • Your Date of Birth
     • Your identifiable and clear photograph
5. Attach Self attested copy of the Address proof (in case of companies/ firms etc. the attestation should carry the required Rubber Stamp). Click here for valid list of Address proofs.
   • Address proof should clearly depict
     • Your name in Full
     • Address in Full (including City, Pin code number etc.)
     • Should not be older than 1-2 months, if you are providing electricity bill, bank statement etc.
     • Should be valid, if you are providing Passport, Driving License etc.
6. Make the Payment online using a credit card/ netbanking or by Cheque/DD payable at Bangalore. The cheque/DD should be drawn in favour of  "eLagaan.com"
7. Send all the documents alongwith details of online payment/ Cheque or DD to address provided here.
