How to find 'Assessing Officer code'?

PAN (Permanent Account Number)

Assessing Officer code may be obtained from Income Tax Office where you submit your return of income. Applicants who have never filed return of income may find out Assessing Officer code with the help of IT PAN Service Center or TIN Facilitation Center or jurisdictional Income Tax Office.

About Challan Head for Registration of Valuer(WT),Form-N of 34AB

Dear Sir/Madam
With Reference to above subject it is to inform you that I want to be a registered Valuer (under section 34AB of the wealth Tax Act 1957) of Income Tax. I am residing at Ambikapur, Distt-Surguja, State- Chhattisgarh. We fail to know on Which Head We make a SBI challan of 1000/- for registration fees. I am all ready getting Register Fellow of Instituation Of Valuers , so plese reply the following--
1- On which Head of Chhallan & Whom ,is it AO ?
2- Where we submitte the Form N (Rule 8B)of ambikapur, Chhattisgarh.
3- Is it necessary to send Original Certificat along with photo copy ?

So that I can apply for a PAN number, I need a code of AO by default. Please let me know the default code AO Trichy 620,018.
